We’re kick-starting a campaign that gives voice to our hospitality celebrities, from small retail shop owners to housekeepers. Executive chefs to concierges. Hotel managers to museum curators. If your job helps someone else enjoy our city, you help power our hospitality industry.

By submitting this video, I hereby agree and consent to the use by Destination Madison Foundation, Inc. (“Destination Madison”) and its agents and affiliates of my name, picture, and likeness in connection with its business and promotional purposes.
I also grant to Destination Madison the right to publish, use, edit, and/or adapt my submitted content. This may include, for example, adapting the image, video, or content from it for posting on a website (in either video or still format), and preparing still images and text excerpted from the content. Accordingly, I hereby assign and transfer to Destination Madison all rights, title, and interest, including any copyright that I may have in the above video, image, or content. If for any reason this transfer of rights is void or limited by law, I grant to Destination Madison and its agents and affiliates an irrevocable license to publish, use, edit and/or adapt the image or video above and its content, for use in any and all media, without limitation, and without payment or any other consideration. Destination Madison may use, edit, publish, and distribute the above image or video in whole or in part without restriction as to changes or alterations and I hereby waive any right to review, inspect, or approve any such edits, modifications, or alterations.
I hereby forever release and discharge Destination Madison, its officers, directors, owners, agents, representatives, employees, affiliates, and their successors and assigns (collectively the Released Parties), from any and all actions, obligations, costs, damages, losses, claims, liabilities and demands of whatever kind or nature that I had, now have or hereafter may have against any one or more of the Released Parties, arising from, related to or in any way connected with my submission.
